2. Are you traveling with non-diving companions or children?
3. If you are traveling with children, please advise the age of the children?
4. What is the name of the certification agency, training level and experience level (number of dives) date of most recent dive, for each member of your party?
5. Do you require equipment?
6. Are you interested in day trips or overnight stays in semi remote locations?
7. How many dives would you like to complete during your stay?
8. Do you require accommodation? (If so what style, budget, configuration.)
9. Do you have any special interest areas?
10. When do you expect to be in Bali?
11. Special requests
